Please print both forms (Ctrl P to print 2 pages) and take the Impression Guide to your hearing professional when you have your ear impressions taken. Have the hearing professional use the check list in the guide. Fill out the Order Form and send both forms, with your ear impressions, to our lab, using the mailing label.
